This is the FILLABLE AND SAVEABLE template for the Record Book required at the D3 and up rating levels.   On the USPC website under Forms the "Record Book Guide, Health and Maintenance" will provide an example of how this should be filled out and why.  For the D3, three months or more of records are required; the C1 rating requires 6 months or more; the C2 rating requires 9 months or more of records.  The depth of knowledge included in the Record Book is expected to increase as the rating level increases.
