This is a required form for Show Jumping Rally.

This is a list of required equipment each team must have for rallies.  Don't go buy everything on the list!  Each team will bring one of each of the kits and typically these are divided up among team members.  Individual equipment for the competitor and their mounts will have to be brought by each person on the team.  REMEMBER TO CHECK EXPIRATION DATES ON FIRST AID KIT ITEMS!

Stall Cards are required for rallies and some ratings.  As long as the information on the card is the same as what is shown on this USPC formatted card, teams may individualize stall cards for rally.  The card should reflect the depth of knowledge appropriate to the rating of the rider.

This medical card is worn in the armband of participating members at all mounted and unmounted pony club activities.  The card is double-sided with medical information on the front and release signatures on the back.  The card should be folded so top front center with participant name shows through the armband case.

Each team at rally is required to have one chaperone who is present on the rally grounds for the duration of the rally.  This form details the chaperone duties and includes a signature page for submittal with the team's paperwork.

The Activity/Rally Release is a required form for every rally.  Only the sections applicable to the rally for which it is being submitted need be filled out.  For SJ rally where the competitor is riding in a non-qualifying division, simply write in the division name.  For Rating-Level Eventing Rallies (D2 or D3) write in the applicable levels under Other in the Eventing section.  Don't forget all three pages of the form!  Required signatures: One parent signature on page 2; two parent signatures on page 3; two pony clubber signatures on page 3; and one parent signature on page 4. Pages 2-4 are the pages that need to be returned (1 and 5-6 are informational pages).
